The Office of Vital Statistics/Registrar maintains records on births, deaths, and marriages within the City of Providence. This department also provides certified copies of these records – for a fee – and is also responsible for the issuance of marriage licenses.
Application forms are available below (in PDF format) for you to download, fill out, and hand deliver or mail to the Department of Vital Statistics.

- A copy of a valid picture ID MUST be submitted along with the application with all mail requests.
If you do not have a valid state ID, and are seeking a vital record, please bring any TWO (or any one item twice) from the list below. Attach a photocopy of BOTH documents to the application. Both documents must include the name and address of the applicant.
