This survey is intended for restaurants enrolled in the third-party delivery service DASAP (“Delivery As Soon As Possible”) as part of a program through the City of Providence and District Hall.
The survey has 5 brief sections, and takes 10-15 minutes to complete:

You are not required to complete the survey to continue in the DASAP delivery program. Your responses will help District Hall create trainings that best meet your needs.
We ask for information on sales and the cost of third-party delivery programs (Seamless, GrubHub, etc.) to help the City measure the benefit of offering this program. It will not be shared beyond the Department of Innovation or used for any other purpose.
