Beginning the application process online will make your in-person appointment faster and easier! After applying online, you will need come to the Vital Records Office (Office 104, 25 Dorrance Street) with accepted forms of identity, proof of residency and payment within 30 days of completing this online application. You must have/be:
- Two forms of proof of residency (you must be a Providence resident)
